Arendell Parrot Academy

September 28, 2009

Dear Parent:

The Board of Trustees recently appointed a long-range planning committee to consider relevant issues and future needs of Parrott Academy.  The Committee is comprised of representatives from the following categories: parents, students, faculty, administration, and Board of Trustees Members.  The goal of the Committee is to present to the Board of Trustees a 10-year plan of action for the school that will be used as a road map for school enhancements and improvements.

In an effort to identify current issues and future needs of Parrott Academy, parents, students, alumni, and teachers will be asked to complete a survey.  All surveys will be completed on an anonymous basis
.

You may complete the parent survey on-line or on a hard copy.  A link will be sent to all parents who are on the school's e-mail list.  There also will be a link on the APA home page.  If you would prefer to complete the survey on a hard copy, you may request a copy of the survey by responding to the e-mail transmitting the link or by requesting a copy from the office.  Hard copies of the survey may be returned to the office in person or by mail.

Our goal is for
each parent (mother and father) to complete a separate survey.  We recognize that the survey will take some time to complete.  We urge you to take time to respond.  This is your opportunity to impact the future of Parrott Academy.

The survey period will be from October 2 to October 18.  After the results have been tallied, we will host focus group meetings in the different areas.  You will be receiving more information on the focus groups in the near future.

This is your opportunity to make a difference!  Thank you in advance for your participation.

                                      Yours very truly,

                                         Dr. Steve Mazingo

If you come across a question in the survey that asks about a child’s specific experience and you have more than one child at our school, please answer the question with the child who has been at our school the longest in mind.

Below are a few other things to keep in mind as you progress through the survey:

1.  Once you begin the survey you will need to complete it in one sitting.

2.  You must complete all of the required questions in order to advance through the survey. Required questions are marked with a red asterisk.

3.  As you move from page to page in the survey, please use the Previous and Next buttons at the bottom of each survey page. If you use the back button on your browser tool bar, it will cause you to exit from the survey and you will have to re-enter the online tool and begin again.

4.  If you use a wireless connection to the internet, please be aware that slight disruptions can cause that connection to break and sever your connection to the internet. Generally, you will know that this has happened when you click on the Next button to advance to another page in the instrument and it asks you for a log-in and/or password. Unfortunately, if this happens, you will have to begin the survey again.

5.  You may not complete more than one survey.

6.  If you would prefer to complete a hard-copy of the survey, please email Cindy Murphrey at cmurphrey@parrottacademy.org or contact the front office.

Thank you for taking the time to complete this very important assignment.  We hope to hear from you!
